JULY 2007Family Health International, with support from the India AIDS Initiative (Avahan) of the Bill & Melinda Gates Foundation, developed the Clinic Operational Guidelines and Standards (COGS) as an essential tool for ensuring standard and quality STI services for sex workers. COGS define the minimum standards for STI services beyond STI case management. They form the basis for training and supervision, and serve as a benchmark against which the performance of individual clinics can be monitored. The manual is intended for STI program managers, technical teams, and clinic and outreach teams involved in STI-service provision for sex workers and other high-risk populations.

The guidelines and standards concern STI clinic operations (clinic set-up, staffing, community involvement, coordination with outreach, and logistics support). They also address clinical management, with detailed protocols for the sex-worker "essential package"syndromic case management, regular STI screening, asymptomatic STI treatment, and standards on comprehensive STI management. In addition, they deal with health education and counseling, including reproductive health issues; STI laboratory support; infection control; ethical standards, confidentiality, and right of refusal; and monitoring, evaluation, and reporting.
