HIV Care in Low-Resource Settings and the accompanying document of tools can help organizations and communities in low-resource settings create effective health referral networks for persons living with HIV/AIDS and their families and caregivers.  Referral networks can improve delivery of various services in comprehensive HIV prevention, care, treatment and support.

The companion document discusses different forms and tools, such as referral forms and referral registers. Having tools standardized and available to organizations in the referral network is critical to maintaining accuracy, efficiency and consistency. These sample tools can be downloaded here in Microsoft Word and adapted for different settings. Each tool is accompanied by instructions that describe how it is designed to be used.
